The Pain Practitioner


Myofascial pain arises from muscle and its connective tissue. This noninflammatory condition is distinguished from other soft tissue disorders such as fibromyalgia, tendonitis and bursitis. It presents with regional pain, often present in select quadrants of the body, and is accompanied by increased tension and decreased flexibility in the related muscle and fascia. Myofascial pain may present independently but is often a component of many acute and chronic pain conditions. Critical in its diagnosis is the presence of one or more myofascial trigger points — discrete, hyperirritable nodules located in a taut band of skeletal muscle that are palpable during physical examination.

Recent studies on chronic low back pain rehabilitation suggest that predictors of treatment outcome may be different according to the considered conservative treatment. The aim of this study was to identify predictors of response to back school, individual physiotherapy or spinal manipulation for cLBP.

Low back pain is common, costly and a significant cause of long-term sick leave and work loss. Many different interventions are available, but there exists no consensus on the best approach. One intervention often used is manipulative therapy. Current evidence from randomized controlled trials demonstrates that manipulative therapy may be as effective as other conservative treatments for LBP, but its appropriate role in the healthcare delivery system has not been established. The objective of this study was to assess changes in pain levels and physical functioning in response to standard medical care vs. SMC plus chiropractic manipulative therapy for the treatment of low back pain among 18- to 35-year-old active duty military personnel.

Manual Therapy Journal


Spinal mobilizations are a common form of treatment intervention applied by physiotherapists in clinical practice to manage musculoskeletal pain and/or dysfunction. Previous research has demonstrated that mobilizations cause a hypoalgesic effect. However, there is very little research investigating the optimal treatment dose inducing this effect. The aim of this study was to investigate the effect of the number of sets (up to 5) and different durations (30 vs. 60 s) on pressure pain thresholds at different sites.
